Apply for your Medical Certificate

Follow the steps below to complete your medical certificate application

Application Process

  • Step 1: Download and Fill out medical Form CG-719K/E and drug test Form CG-719P

    • Download the Form CG-719K/E (Medical Examination Report) and drug test from

  • Step 2: Schedule and complete Medical Exam and Drug Test

    • Find nearby clinic that can complete DOT or USCG physicals (Google search "USCG or DOT Physical Near Me")

    • Bring the CG-719K/E and CG-719P form to your appointment.

  • Step 3: Complete the Medical Exam and Drug Test

    • The doctor will evaluate your:

      • Vision and hearing.

      • Physical fitness (e.g., mobility, strength, and cardiovascular health).

      • Mental health and cognitive abilities.

    • The doctor will complete and sign the CG-719K/E and CG-719P form.

  • Step 4: Submit the completed CG-719K/E form ONLY to the USCG National Maritime Center (NMC) online or by mail.

    • Online (preferred method): Email scanned application as a PDF to MEDAIP@uscg.mil

      • Subject line of email: ​Last name, First name​

    • Mail: Send to the NMC address listed on the form.

  • Step 5: Keep CG-719P (Drug test form) separate. It will be submitted with MMC Application separately​

  • Q: What is a TWIC card?
    A security ID for accessing secure areas of ports and ships.
  • Q: How do I get a TWIC card?
    Apply through the TSA, complete a background check, and pay a fee.
  • Q: Who needs a TWIC card?
    Maritime workers, truckers, and others needing unescorted access to secure port areas.
  • Q: How much does it cost?
    $124 for new applicants (valid for 5 years).
  • Q: What’s the wait time?
    Usually 2-3 weeks after your in-person enrollment appointment.
  • Q: How long is it valid?
    5 years.
