Apply for your Merchant Mariner Credential (MMC)

Follow the steps below to complete your medical certificate application

Step 1: Download and Fill out MMC Application form CG-719B
- Download the MMC Application CG-719B and fill out with all information requested
  - Application Guide:
    - Section I: Fill out the personal information that is requested in the form
    - Section II: Check the box "Entry Level" under "Original" Only
    - Section III:
      - Part 1: Skip this section unless you are exempt from a TWIC. (Most are not exempt)
      - Part 2 and 3: Check applicable boxes for criminal record and NDR.
    - Section IV
      - Part 1: Check yes or no for the outreach program. It is optional.
      - Part 5: Signature and Signature
      - Part 5b: Authorization of Oath (can be completed by Notary Public, Judge, or anyone allowed to administer an Oath)
      - Part 6: Skip unless requested by third party
    - Print name at the bottom right of each page of application
Step 4: Submit the completed application (CG-719B form) to the USCG National Maritime Center (NMC) online or by mail.
- Online (preferred method): Email scanned application as a PDF to MMCApplications@uscg.mil.
  - Subject line of email: Last name, First name
- Mail: Send to the NMC address listed on the form.
